Completed and signed applications should be given to the troop committee chairman. Please note that adult applications have a separate signature page that must accompany the application.

The Boy Scouts of America
requires an annual physical by a health-care professional
in order to participate in Scouting activities, long term
camps, Jamboree, and high adventure bases. The following
